Job summary

CareOne Home Health is seeking a Licensed Physical Therapist for home health services in Monterey County and surrounding communities. This per diem role offers mileage reimbursement and potential full-time growth as the agency expands.

You will provide one-on-one care in patients' homes, focusing on orthopedic, respiratory, cardiac and other diagnoses, tailoring plans to individual goals.

Qualifications

• Licensed Physical Therapist with CA license and current CPR/BLS.
• Experience in home health settings preferred.
• Knowledge of PT evaluation and care planning.

Responsibilities

• Initiate PT evaluation and care plan; instruct family/caregivers.
• Educate patient for prosthetic devices as applicable.
• Prepare and submit clinical/progress summaries using standards.
• Participate in start of care and discharge planning.
• Provide PT consultation to families/caregivers as needed.
• Offer in-service education for nursing staff as needed.
• Join interdisciplinary teams and quality improvement activities.
• Supervise PT assistants when applicable.
