Home Health Visits - Physical Therapist

BAYADA is currently seeking an experienced Physical Therapist, PT, for an opportunity performing home health visits with our adult and senior clients located throughout Middlesex and/or Monmouth County.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wounds and promote wound healing using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary team as appropriate.
• Educate and instruct clients, family members, or caregivers in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist:

• A current New Jersey Physical Therapist (PT) license.
• At least one year of recent clinical experience as a Physical Therapist.
• Graduation from an American Physical Therapy Association-approved program or equivalent, as evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.

Base Pay, depending on qualifications and negotiations:
Starting at $60+ per visit point
