Job Summary

The Physical Therapist provides rehabilitative services to elderly/frail participants in a PACE (Program of All-Inclusive Care for the Elderly) setting. The role is both clinical and leadership-focused, supporting the interdisciplinary team (IDT) and ensuring optimal function and safety for patients.

Perform assessments and develop care plans with IDT
Supervise therapy assistants
Train staff and caregivers in therapeutic equipment use
Coordinate with OT and IDT for seamless care delivery
Support participant enrollment and end-of-life care
Participate in quality improvement and compliance initiatives
5+ years of PT experience (2+ years with elderly/frail population)
2+ years supervisory experience
Strong leadership, clinical, and communication skills
Experience in fast-paced and culturally diverse environments
Must be self-directed and data-driven
Required: Bachelor’s in Physical Therapy (U.S. equivalent if foreign)
Preferred: Master’s or Doctorate in PT (U.S. equivalent if foreign)
