TREK Healthcare is seeking a dedicated and compassionate Home Health Physical Therapist (PT) to join our team. The PT will be responsible for evaluating, planning, and implementing physical therapy treatments for patients in their homes to improve mobility, strength, and independence. This role requires strong clinical skills, adaptability, and a passion for patient-centered care. PT will be seeing patients all throughout the Bitterroot.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations for patients in their homes.
- Develop and implement individualized treatment plans to improve mobility, balance, and strength.
- Provide hands-on therapy and teach exercises to help patients regain functional independence.
- Educate patients and caregivers on home exercises, safety precautions, and injury prevention.
- Monitor and document patient progress, adjusting treatment plans as needed.
- Collaborate with physicians, nurses, occupational therapists, physical therapy assistants and other healthcare professionals to coordinate care.
- Adhere to all state and federal regulations, as well as company policies and procedures.
- Ensure timely and accurate documentation in the EMR system.
- Provide compassionate and professional care to facilitate a supportive healing environment.
Qualifications:
- Education: Doctorate (DPT) or Master’s degree in Physical Therapy from an accredited program.
- Licensure: Active and unrestricted Physical Therapy license in the state of Montana.
- Certifications: CPR certification required.
- Experience: Previous home health experience preferred but not required. New grads are welcome to apply!
- Skills: Strong assessment and treatment planning abilities, excellent communication and interpersonal skills, ability to work independently and manage time effectively, proficiency with electronic medical records (EMR) systems, familiarity with OASIS charting components, efficiency with documentation deadlines and state compliance requirements.

- Ability to travel to patients’ homes within the assigned territory.
- Proficiency with Microsoft teams and calendar for effective communication.

Job Type: Full-time
Pay: $52.00 - $60.00 per hour
Expected hours: 32 – 40 per week
Work Location: On the road
