Job summary

BrightStar Care in Texas is seeking a licensed Physical Therapist to deliver one-on-one home health services across the DFW area. You will design and implement personalized rehabilitation plans in patients' homes, adapting to their daily routines.

This PRN role offers the flexibility to set your schedule around availability while collaborating with our clinical team to ensure safe, effective care and progressive outcomes for clients.

Key Responsibilities:

1. Assess patients' physical abilities and plan individualized rehabilitation programs.
2. Implement and monitor treatment plans to ensure effective progress toward goals.
3. Provide education and support to patients and their families regarding treatment protocols and home exercises.
4. Document patient progress and communicate effectively with clinical staff and other healthcare providers.
5. Work collaboratively with our team to provide high-quality, patient-centered care.

Qualifications:

You're a great fit if you have the following skills.

1. Valid Physical Therapist license in the state of Texas.
2. Experience in home health or outpatient settings.
