Meriden Healthcare & Rehab is now hiring a Physical Therapist (PT) to provide rehabilitation services to our patients and residents. We are seeking a caring, warm-hearted Physical Therapist who is currently licensed in Connecticut or is a graduate from an approved Physical Therapist program and is actively working toward obtaining licensure. Join our compassionate team of rehabilitation professionals dedicated to helping patients recover their strengths, skills, independence, and confidence to return to their highest level of function and an active life.

Responsibilities:

• Develop and implement physical therapy programs that focus on rehabilitation per state practice act.
• Work directly with patients to achieve maximum physical recovery.
• Evaluate effects of therapy treatment and communicate patients' progress.
• Record and document patient care services accurately and concisely.
• Collaborate with other team members to provide comprehensive care.

Qualifications:

• Graduate of an accredited Physical Therapy Program.
• Current CT State PT License.
• Experience in SNF settings is preferred, but not required.
