Job summary

AOM Healthcare is hiring a Full-Time Physical Therapist at Ridgewood Manor in Maumee, OH (3231 Manley Rd) with a shared position to Addison Heights on Butz Rd. You will assess patients, develop and update plans of care, and work closely with Physical Therapist Assistants and the care team to maintain high-quality rehabilitation services.

We seek a dedicated professional with Ohio PT licensure, strong patient-centered care, and a collaborative spirit.

Qualifications

• Current licensure as a Physical Therapist in Ohio.

Responsibilities

• Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants.
• Assesses patients’ needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork
• Observes and evaluates treatment effect, accomplishment of short and long term goals. Recommends change to physician, if needed
• Performs an appropriate assessment on each patient and performs reassessments.
• Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention
• Formulates and updates patient specific plan of care for physical therapy
• Current licensure as a Physical Therapist in Ohio
• Additional duties as assigned
