Job summary

Fusion Physical Therapy Partners in Manchester, NH seeks a licensed Physical Therapist to join a collaborative, clinician-led private practice network. You will evaluate patients, design individualized care plans, and deliver evidence-based treatments with a focus on improving function and reducing pain.

Qualifications

• Active New Hampshire Physical Therapist license in good standing (or eligibility).
• Graduate of an accredited Doctor of Physical Therapy (DPT) program.
• Demonstrated ability to deliver evidence-based patient care.
• Commitment to continuous learning and professional growth.
• Experience in an outpatient orthopedic setting preferred.

Responsibilities

• Evaluate patients and develop individualized plans of care based on findings and goals.
• Deliver evidence-based interventions to improve strength, mobility, balance and function.
• Utilize manual therapy, therapeutic exercise, and neuromuscular re-education as appropriate.
• Monitor progress and adjust treatment plans to achieve goals.
• Educate patients on diagnoses, treatment plans, home programs, and injury prevention.
• Maintain accurate documentation using clinical software and prompts.

We're seeking a Physical Therapist to join our Manchester, NHclinic! About the Role

As a Physical Therapist at CPTE, you'll help patients improve mobility, reduce pain, restore function, and return to the activities they love. You'll evaluate and treat a diverse outpatient orthopedic population while working alongside a supportive clinical team dedicated to exceptional patient outcomes and individualized care.

Your day-to-day

• Evaluate patients and develop individualized plans of care based on clinical findings, functional limitations, and patient goals
• Deliver evidence-based treatment interventions designed to improve strength, mobility, balance, coordination, endurance, and overall function
• Utilize manual therapy techniques, therapeutic exercise, neuromuscular re-education, therapeutic activities, and other skilled interventions as appropriate
• Monitor patient progress and modify treatment plans to optimize outcomes and support goal achievement
• Educate patients on diagnoses, treatment plans, home exercise programs, injury prevention strategies, and sustainable movement strategies tailored to each patient's life
• Maintain accurate and timely documentation using Prompt and other clinical technologies
• Collaborate with fellow Physical Therapists, Physical Therapist Assistants, Athletic Trainers, front office staff, referral sources, and interdisciplinary providers to deliver an exceptional patient experience
• Participate in ongoing professional development through mentorship, continuing education, and clinical collaboration opportunities
• Build meaningful relationships with patients and colleagues to support a positive, team-oriented clinic culture
