The Office for Children with Special Health Care Needs (OCSHCN) is seeking a Physical Therapist in Jefferson County. OCSHCN provides comprehensive services for children and youth with special health care needs through early intervention, pediatric specialty clinics, care coordination, case management, therapies, and collaborations.
OCSHCN is an agency within the Cabinet for Health and Family Services (CHFS). This position will report to the office for the initial probationary period.
Responsibilities Include But Are Not Limited To
- This position addresses the physical, cognitive, psychosocial, sensory-perceptual, and other aspects of performance in a variety of contexts and environments to support engagement in activities that affect physical and mental health, wellbeing, and quality of life. Works as part of an interdisciplinary team to provide a treatment plan for medically complex pediatric patients.
- Provides individual evaluations, assessments, and treatment to address areas of gross motor movement, range of motion, strengthening, and activities for daily living. Plans physical therapy treatment programs and goals for patients with disabilities. Prepares evaluation plans in collaboration with caregivers and other professionals. Communicates and uses professional documentation standards.
- Performs initial and annual evaluations. Performs direct physical therapy treatment such as relaxation, muscle strengthening, prevention and reduction of contractures, developmental/age-appropriate activities, and other therapeutic exercise plans. Writes home and school programs and instructs families in the home treatment plan.
- Attends the pediatric specialty medical clinics in the designated region, receives referrals from professional staff, and evaluates and consults on patients' physical therapy needs. Documents patients progress. Evaluates and suggests proper equipment and the modification of equipment. Acts as a liaison with school physical therapists and professionals. Contacts insurance companies or Medicaid to accomplish goals for patients and secure appropriate therapy/medical equipment. Collaborates in the care coordination and transition planning with office staff and community partners and documents in the medical record. Contacts other professionals on behalf of the patient, such as a teacher, school physical therapist, or durable medical equipment provider.
- Encourages knowledge of, respect for, and development of skills to engage with those of other cultures or backgrounds. Represents the Office for Children with Special Health Care Needs at meetings, mentors students, and provides outreach and education.
- Performs other duties as assigned.

- Ability to multi-task and delegate.
- Strong verbal and written communication skills, time management and organizational skills are necessary.
- Experience in performance and operations management
- Proficient in MS Office 365.

Minimum Requirements
EDUCATION: Successful completion of the academic and clinical requirements of a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). http://www.capteonline.org

Working Conditions
