Job summary

Saint Joseph Hospital - Lexington seeks a Physical Therapist to help patients regain or maintain function, reduce pain, and improve quality of life. You will perform evaluations, craft plans of care, and deliver varied treatments while meeting productivity and collaborating with families and the care team.

Strong communication, patient-centered care, and age-appropriate skills are essential. A strong focus on documentation and regulatory compliance will be maintained.

Qualifications

• Master's in Physical Therapy from an APTA-accredited program.
• KY licensure upon hire.
• Student or prior experience in a rehab setting.
• BLS/CPR within 14 days.

Responsibilities

• Perform initial evaluations and develop individualized plans of care.
• Provide a variety of direct treatments and track progress.
• Collaborate with interdisciplinary team and families; maintain productive care.
• Document all care and maintain records per Joint Commission, CARF, Medicare, and regulations.
• Communicate therapy programs and departmental issues effectively.

Job Summary and Responsibilities

As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.

Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.

To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.

• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
• Evaluates assigned patients within the guidelines established by the physician and in compliance with departmental policies and procedures.
• Implements competent treatment and performs regular reassessments for the purpose of treatment modification, to maximize patient’s potential for functional independence, all in accordance of departmental standards.
• Completes all required documentation and maintains pertinent records of all assigned patients, in order to be in compliance with Joint Commission, CARF, Medicare, and all other regulatory requirements.
• Demonstrates the ability to be efficient and productive by organizing job duties and responsibilities.
• Maintains effective communication regarding Therapy programs and departmental issues.
