Lake Village Nursing & Rehabilitation in Lewisville, TX seeks a Full-Time Physical Therapist to provide inpatient and outpatient care within our in-house rehab program.

The role requires a valid Physical Therapist license; new grads are welcome. You will screen and evaluate patients, develop individualized care plans, deliver rehabilitation interventions, wound care, and comprehensive discharge summaries while collaborating with families and care teams to drive recovery.

Responsibilities

• Deliver rehabilitation interventions to patients per regulatory standards.
• Administer procedures as part of rehab plan including manual techniques, ambulation, exercises, modalities, assistive devices, and wound care.
• Provide consultation and counseling to patients and caregivers regarding physical disorders.
• Generate discharge summaries ensuring regulatory and clinical compliance.

Inpatient and Outpatient Care

Lake Village Nursing & Rehabilitation in Lewisville, TX is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist to provide inpatient and outpatient carefor our in-house rehab program!

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required. Open to Physical Therapists at all experience levels. New Grads are welcome!
