Job summary

Duly Health and Care is seeking an outpatient Physical Therapist in Lemont, IL. This role emphasizes one-on-one patient care, structured mentorship, and opportunities to grow clinically within a strong internal referral network.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Illinois Physical Therapist license.

Responsibilities

• Provide outpatient PT care with one-on-one patient sessions.
• Perform comprehensive evaluations and develop individualized treatment plans.
• Collaborate with PTs, OTs, PTAs, physicians and other providers.
• Maintain schedule: Monday–Friday with evenings, last patient at 7:00 PM.
