Job summary

Lake City Healthcare and Rehabilitation in Lake City, FL is seeking a dedicated Physical Therapist to join our rehabilitation team on a PRN basis. Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist license/registration.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct physical therapy services per regulations and facility guidelines.
• Collaborate with administration, physicians, nursing staff, and interdisciplinary team members to coordinate therapy services.
• Perform comprehensive patient evaluations including ROM, manual muscle testing, gait analysis, and functional assessments.
• Develop and administer individualized PT treatment plans to restore function, relieve pain, and prevent disability.
• Monitor resident progress and adjust treatment plans to meet therapy goals.
• Complete accurate and timely documentation including evaluations, plans of care, notes, and progress reports.
• Maintain professional standards and ensure compliance with therapy policies and regulatory requirements.
• Provide support and guidance to therapy staff while promoting collaborative resident care.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.

Key Responsibilities
