Lake City Healthcare and Rehabilitation is seeking a dedicated and experienced Physical Therapist to join our rehabilitation team. The Physical Therapist will provide high-quality therapy services to residents while developing and implementing individualized treatment plans focused on improving mobility, restoring function, reducing pain, and enhancing overall quality of life.
The ideal candidate will have experience working in skilled healthcare settings and a passion for helping residents achieve their highest level of independence through compassionate, evidence-based care.
Plan, develop, organize, implement, evaluate, and direct physical therapy services in accordance with current regulations, standards, and facility guidelines.
Collaborate with administration, physicians, nursing staff, and interdisciplinary team members to coordinate effective therapy services.
Perform comprehensive patient evaluations, including range of motion assessments, manual muscle testing, gait analysis, and functional assessments.
Develop and administer individualized physical therapy treatment plans to restore function, relieve pain, and prevent disability.
Monitor resident progress and adjust treatment plans as needed to achieve therapy goals.
Complete accurate and timely documentation, including evaluations, plans of care, clinical notes, and progress reports.
Maintain professional standards and ensure compliance with therapy policies, procedures, and regulatory requirements.
Provide support and guidance to therapy staff while promoting a collaborative approach to resident care.
Participate in interdisciplinary team meetings and contribute to overall quality improvement initiatives.
Bachelor’s Degree in Physical Therapy from an accredited college or university.
Current Physical Therapist license and/or registration in the state of Florida.
Experience in skilled nursing, rehabilitation, hospital, or healthcare settings preferred.
Strong communication, organizational, and clinical skills.
Ability to work effectively as part of an interdisciplinary team.
