Key Responsibilities

• Develop and update individualized care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.

Requirements

• Licensed Physical Therapist in the State of Nevada.
• Degree from a CAPTE‑accredited program; passed state‑approved exam.
• Minimum 2 years’ experience (home health preferred).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.
