Creative Solutions In HealthCare is hiring a Full-Time Physical Therapist to join our in-house team providing person-centered care to residents and helping achieve positive outcomes.

You will assess, plan, and administer therapies, collaborate with an interdisciplinary team, and ensure care follows established plans and regulatory requirements. In-house support and competitive benefits accompany flexible scheduling.

Qualifications

• Active Texas license in Physical Therapy in good standing.
• SNF/Long term care experience preferred.
• Current knowledge of physical therapy practices.
• Ability to manage patients with different personalities.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs for patients.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Ensure all treatment follows the plan of care.
• Monitor patient response and adapt approach with IDT input.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Ensure timely, thorough documentation compliant with regulations.
• Comply with all rules and procedures.
• Attend required meetings designated by Rehab Director.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Complying with all rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

Requirements

• Active/Valid Texas license in Physical Therapy.
• In good standing with all regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.
