Duties

• Conduct assessments to develop individualized treatment plans
• Implement therapeutic exercises and modalities to improve patients' mobility and function
• Educate patients and their families on home exercise programs
• Collaborate with healthcare professionals to coordinate care plans
• Perform discharge planning and follow-up care as needed
• Document patient progress accurately and timely
  
  

Requirements

• Degree in Physical Therapy from an accredited program
• State licensure as a Physical Therapist
• Experience in outpatient, acute care, or home health settings preferred
• Proficiency in developing care plans and discharge planning
• Knowledge of orthopedics, geriatrics, and trigger point therapy
• Strong interpersonal and communication skills
