provides high-quality outpatient rehabilitation through both in-clinic and virtual care.

Key Responsibilities

• Conduct comprehensive patient evaluations and assessments
• Develop evidence-based treatment plans targeting function, mobility, and quality of life
• Deliver hands-on physical therapy and incorporate digital health tools (telehealth and remote monitoring)
• Document evaluations, interventions, and patient progress
• Collaborate with a multidisciplinary team to coordinate care
• Educate patients and families on treatment goals and self-management
