Job summary

Cornerstone Rehab is seeking a PRN Physical Therapist to deliver care to adult and geriatric patients across nearby centers. You will collaborate with a diverse team of clinical professionals and contribute to patient outcomes through skilled therapy and documentation.

The position requires an active State PT license and SNF or acute care experience, with travel to different facilities as needed. Cornerstone Rehab supports clinician development and comprehensive patient care.

Qualifications

• Valid State PT License and credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
