Job summary

Cornerstone Rehab is seeking a PRN Physical Therapist to deliver high-quality care to adult and geriatric patients within our skilled nursing and post-acute settings.

You will prepare documentation, discharge summaries, and plans of care while collaborating with a diverse team of clinicians; occasional travel to nearby centers is required to support patient outcomes.

Qualifications

• Must hold a valid state Physical Therapist license.
• Experience in SNF or acute care settings preferred.

Responsibilities
