Job summary

Anew Home Health is seeking a licensed Physical Therapist to join our Indianapolis area team. This full-time opportunity covers the south side of Indianapolis and surrounding areas, with PRN hours available. We prioritize compassionate service, integrity, and staying connected with patients in their homes.

You will implement, evaluate, and adjust treatment plans, discharge plans, and participate in case management to deliver high-quality care consistent with agency policy and state regulations.

Qualifications

• Evaluates clients and develops initial and ongoing treatment plans.
• Develops discharge plans and contributes to case management.
• Provides quality patient care in accordance with agency policies and procedures.
• Adheres to state regulations concerning physical therapy and refers/supervises patients for treatment to PTA.

Responsibilities

• Implements physical therapy services with minimal supervision.
• Evaluates clients and develops treatment plans.
• Develops discharge plans and contributes to case management.
• Coordinates with PTAs and adheres to regulatory guidelines.
• Supports patients to live well at home during treatment.

The Physical Therapist implements, with minimal supervision, quality physical therapy services for patients in accordance with the principles and practices of physical therapy and within agency policies and procedures. Evaluates clients, develops initial and ongoing treatment plans, develops discharge plans, contributes to case management and provides quality patient care. Adheres to applicable state regulations concerning physical therapy and refers/supervises patients for treatment to physical therapist assistant (PTA).
