Job summary

Angels Care Home Health seeks a dedicated Physical Therapist (PT) to plan, implement, and evaluate patient care plans to restore and maintain patient well‑being in Imperial, NE.

You will develop PT treatment programs based on medical referrals and evaluations, and collaborate with an interdisciplinary team to optimize home health outcomes. A state license, CPR certification, and DME knowledge are required.

Qualifications

• Must hold current state Physical Therapist license.
• Minimum two years of clinical PT experience.
• CPR certification and Hepatitis profile current.
• Knowledge of durable medical equipment required.
