Job summary

Amenity Health Services is seeking a Physical Therapist to deliver quality care per physician orders. You will evaluate, treat, and plan care for patients, coordinating with a multidisciplinary team and families.

A strong customer service approach is required to support pediatric patients and their families. The role emphasizes maintaining documentation, adherence to standards, and developing care plans while building strong relationships with patients, guardians, and staff.

Qualifications

• Master's or Doctoral degree in Physical Therapy.
• Experience with pediatric patients and/or pediatric patients with developmental disabilities.
• Great customer service skills.

Responsibilities

• Provide Physical Therapy services as prescribed by the patient's physician.
• Maintain a caseload of at least 33 visits weekly.
• Write a written evaluation defining PT needs, goals and outcomes with the referring physician's plan of treatment.
• Develop a written plan of care for Physical Therapy with goals and outcomes.
• Maintain and adjust the plan of care as needed for assigned patients according to PT goals.
• Maintain patient and company confidentiality and coordinate with the healthcare team and family.

Patient care may involve evaluations, administering prescribed treatments and therapies, care planning, communication with other members of the patients multi-disciplinary team, coordination of care, health promotion and teaching and training family members.**Key Responsibilities*** Provide Physical Therapy services as prescribed by the patients physician* Maintain a caseload/productivity of at least 33 visits weekly* Provide a written evaluation defining Physical Therapy needs, goals and expected outcomes in conjunction with referring physicians plan of treatment* Follow policies and procedures and adheres to all applicable patient care standards, licensure regulations and state laws* Develop a written plan of care for Physical Therapy with reasonable goals and outcomes* Maintain and adjust the plan of care as needed for assigned patients according to identified Physical Therapy goals and outcomes* Maintain patient and company confidentiality* Maintain effective communication between staff, healthcare team members, and family* Participate in coordination of services (including discharge planning), with health care team, physicians, patient/guardians, nurses and others**Benefits**Benefits are offered to full time employees who work 30 hours or more a week.
