The Staff Physical Therapist evaluates and treats residents, develops individualized treatment plans, collaborates with residents, families, physicians, and interdisciplinary team members, and helps residents achieve greater mobility, independence, and quality of life.This is an excellent opportunity for a PT who values resident-centered care, clinical excellence, teamwork, and professional growth.What You'll DoEvaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.Develop, implement, and update individualized treatment plans focused on mobility, strength, balance, endurance, and functional independence.Provide skilled physical therapy interventions and monitor resident progress toward established goals.Supervise Physical Therapist Assistants (PTAs) in accordance with applicable state practice requirements.Assist the nursing department with training Restorative Aides.Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding resident progress, goals, and discharge planning.Participate in Resident Care Conferences, Utilization Review meetings, rehabilitation meetings, and other interdisciplinary meetings as needed.Provide in-service training and education to facility staff as assigned.Maintain timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention.Recommend and facilitate appropriate durable medical equipment to promote safe mobility and independence, including walkers, canes, splints, elevated toilet seats, and grab bars.Maintain a safe treatment environment by following infection control, safety, and equipment maintenance procedures.Report equipment concerns promptly to ensure all department equipment remains safe and functional.Maintain strict confidentiality of resident and company information.Participate in quality assurance, performance improvement initiatives, and rehabilitation program development as assigned.Build positive and professional relationships with residents, families, staff, and healthcare colleagues.QualificationsCurrent Physical Therapist license in the applicable state.Strong clinical reasoning and communication skills.Ability to work effectively with residents, families, and interdisciplinary team members.Commitment to compassionate, resident-centered care.Strong organizational and documentation skills.Knowledge of skilled nursing and geriatric rehabilitation is preferred.Ability to supervise PTAs in accordance with applicable state regulations.Why Join Hayward Gardens Post Acute?$65.00–$75.00 per hourFull-Time employmentBenefits packageCollaborative rehabilitation and interdisciplinary teamOpportunity to make a meaningful impact on residents' livesProfessional development and continued learning opportunitiesSupportive environment focused on quality rehabilitation outcomesApply Today!If you're a Physical Therapist looking for a rewarding opportunity to provide meaningful rehabilitation services in a supportive post-acute care environment, Hayward Gardens Post Acute would love to hear from you.Join our team and help residents achieve greater mobility, independence, and quality of life!We are an equal opportunity employer.
