Job summary

Hayward Gardens Post Acute in Hayward, CA is seeking a dedicated Physical Therapist (PT) to evaluate residents, develop treatment plans, and help improve mobility and independence in a resident-centered care setting.

You will supervise PTAs, coordinate with physicians and staff, provide education to residents and families, and participate in quality improvement initiatives while maintaining compliant documentation.

Qualifications

  • Current Physical Therapist license in the applicable state.
  • Strong clinical reasoning and communication skills.
  • Ability to work effectively with residents, families, and interdisciplinary team members.
  • Commitment to compassionate, resident-centered care.
  • Strong organizational and documentation skills.
  • Knowledge of skilled nursing and geriatric rehabilitation is preferred.
  • Ability to supervise PTAs in accordance with applicable state regulations.

Responsibilities

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans focused on mobility, strength, balance, endurance, and functional independence.
  • Provide skilled physical therapy interventions and monitor resident progress toward established goals.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with applicable state practice requirements.
  • Assist the nursing department with training Restorative Aides.
  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding resident progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, rehabilitation meetings, and other interdisciplinary meetings as needed.
  • Provide in-service training and education to facility staff as assigned.
  • Maintain timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention.
  • Recommend and facilitate appropriate durable medical equipment to promote safe mobility and independence, including walkers, canes, splints, elevated toilet seats, and grab bars.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance procedures.
  • Report equipment concerns promptly to ensure all department equipment remains safe and functional.
  • Maintain strict confidentiality of resident and company information.
  • Participate in quality assurance, performance improvement initiatives, and rehabilitation program development as assigned.
  • Build positive and professional relationships with residents, families, staff, and healthcare colleagues.
