Job summary

Good Shepherd Rehabilitation in Hamburg, PA is seeking a licensed Physical Therapist to plan, implement, and evaluate rehabilitative care for patients with acute or chronic disabilities.

You will collaborate with the health-care team, assess patient progress, ensure safety, and maintain documentation and adherence to standards. BLS/CPR is required; a Bachelor's degree in Physical Therapy and an active license are essential.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Previous acute care experience or similar setting preferred.
• Physical Therapist license required.
• Basic Life Support / CPR certification required.

Facilitates restoration of function and prevention of disability for patients with rehabilitation needs and/or physical disabilities by planning and administering medically prescribed physical therapy.

ESSENTIAL FUNCTIONS

PROVIDES INFORMATION TO PATIENTS AND HEALTHCARE TEAM

• By meeting them
• providing educational information in conjunction with direct care providers related to treatments, procedures, and continuing care requirements
• answering questions and requests.

MONITORS DELIVERY OF CARE

• identifying process improvements leading to desired care outcomes
• intervening to overcome deviations in the expected plan of care
• reviewing the plan with patients in conjunction with direct care providers
• maintaining ongoing communication with care management regarding variances from the care plan or transfer/discharge plan.
