Role Description

This full-time, on-site Physical Therapist role is based in Grand Rapids, MI. The Physical Therapist will evaluate patients in home and assisted living settings, develop evidence-based treatment plans, and provide direct rehabilitative care to improve mobility, strength, and functional independence. Daily responsibilities include conducting assessments, setting goals, delivering therapeutic exercises and interventions, documenting clinical progress, and coordinating care with other healthcare professionals and caregivers. The role also involves educating patients and families on home exercise programs, safety, and strategies to support long-term recovery and wellness.

Qualifications

• Demonstrated expertise in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans.
• Strong skills in Patient Care and Home Care, including working safely and effectively in home and assisted living environments.
• Solid understanding of Medicine as it relates to musculoskeletal, neurological, and geriatric conditions.
• Current state licensure as a Physical Therapist (or eligibility to obtain licensure in Michigan).
• Doctor of Physical Therapy (DPT) or equivalent accredited degree.
• Excellent communication, documentation, and time-management skills.
• Ability to work independently, adapt to varied care settings, and collaborate within an interdisciplinary team.
• Experience with older adults or home health/assisted living populations is preferred.
