Job summary

Powerback Rehabilitation is seeking a Licensed Physical Therapist to provide patient-centered rehab across post-acute and outpatient settings. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to restore function, mobility, and independence.

You will collaborate with an interdisciplinary team, document in EMR, track outcomes, and educate patients and families to support ongoing progress and engagement.

Qualifications

• Must hold PT license in the state of employment.
• Experience in post-acute and outpatient rehab is preferred.
• Excellent communication and documentation skills.
