Job summary

LiveWell Homecare Agency is seeking a Physical Therapist to provide direct patient care in home environments across community service areas. You will assess patients, plan treatments, and supervise PT practices while educating families.

Role requires a PT license and a valid driver’s license; no prior experience is required. The position includes comprehensive benefits and requires travel within service areas.

Qualifications

• Provides direct patient care activities within a home environment, including assessment and treatment planning.
• Instructs and assists patients with physical activities and use of supportive devices; educates patients and families on procedures.
• Re-evaluates patients and adjusts treatment plans for optimal outcomes; collaborates with physicians and practitioners to coordinate care.
• Evaluates, fits, and adjusts prosthetic and orthotic devices, recommending modifications.
