Job summary

Mercyhealth is seeking a Physical Therapist to evaluate, plan and implement treatment addressing the functional needs of diverse patient diagnoses. The role is onsite in Freeport, IL, with full-time day shifts and some weekend commitment.

The therapist will document thoroughly, code appropriately, and collaborate with multidisciplinary teams to improve patient outcomes. Licensure in WI or IL is required (IL preferred per posting).

Qualifications

• Graduate of an accredited school of Physical Therapy.
• Licensed as a physical therapist in WI or IL.
• CPR/BLS certification within 90 days of hire.

Responsibilities

• Integrates subjective and objective data to establish a treatment plan.
• Utilizes exercise, physical agents, and manual techniques to address deficits.
• Maintains a thorough record of services provided with appropriate coding and billing.
• Communicates with other disciplines and referring providers to enhance outcomes.
• Performs thorough and timely electronic documentation.
• Strengthens care delivery as evidenced by peer review feedback; demonstrates advanced evaluation and treatment techniques.
• Participates in PI and CQI processes.
• Assists in orientation, training and mentoring of new partners.
• Participates in cost-saving initiatives within the department.
• Takes an active role in the performance improvement process.
• Maintains high standard of patient care and effective communication between PTs, assistants and physicians group.
• Plans and conducts lectures and training programs on physical therapy and related topics for medical staff, students and community groups.

Education

Graduate of an accredited school of Physical Therapy

Licensed as a physical therapist in WI, IL or both
