Job summary

Northwoods Rehab is seeking a licensed Physical Therapist to provide comprehensive PT services, assess and treat patients, and collaborate with the rehab team to develop and implement individualized treatment plans.

Responsibilities include conducting screenings, evaluating patients, planning and monitoring interventions, documenting outcomes, educating patients and families, and participating in discharge planning. Travel to home assessments may be required within a reasonable distance.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.

Responsibilities

• Provides and directs physical therapy services to patients, including assessment, treatment planning and implementation, documentation, and communication under physician orders.
• Initiates referrals when appropriate and in accordance with Northwoods Rehab policies and procedures.
• Conducts regular patient screenings to determine need for intervention and treatment.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, families and team members.
• Establishes individual patient schedules with team members.
• Develops treatment plans with short and long-term goals and methods to achieve them.
• Coordinates treatment plans with patient, family and team members.
• Implements treatment as outlined and monitors patient response, adjusting as needed.
• Develops home or community programs to maintain and enhance patient performance at home.
• Documents results of assessments, treatment, follow-up, and termination of services.
• Adapts treatment to meet individual patient needs.
• Provides adaptive devices, orthotics or prosthetics and trains users.
• Participates in discharge planning.
• Supervises Licensed Physical Therapy Assistant, PT Aides, Rehab Techs/Aides, students and volunteers.
• Reviews quality and effectiveness of services using predefined criteria.
• Participates in education for patients, families, and staff.
• Confers with team members to coordinate rehabilitation programs.
• Follows documentation standards and infection control/OSHA standards.
• Maintains professional image and confidentiality.
• May travel to perform home assessments within reasonable distance.

JOB RELATIONSHIPS

Responsible to and reports to the Director of Therapy/Rehab.

JOB RESPONSIBILITIES

1. 1. Provides and directs physical therapy services to patients, including the assessment, treatment, program planning and implementation, related documentation and communication. Functions under physician’s orders.

13. 13. Adapts treatment to meet the individual needs of the patient.
14. 14. Provides, recommends, fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of such equipment.
15. 15. Participates in discharge planning.
16. 16. Supervises and directs Licensed Physical Therapy Assistant in the implementation of the treatment Plan.
17. 17. Supervises Physical Therapy Aides, Rehabilitation Techs/Aides, students and volunteers.
18. 18. Reviews the quality and appropriateness of the total services delivered and of individual physical therapy programs for effectiveness and efficiency, using predetermined criteria.
19. 19. Participates in patient, family and staff education.
20. 20. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient.
21. 21. Adheres to Northwoods Rehab procedures regarding documentation of physical therapy services.
22. 22. Attends in-service meetings and facility meetings as required.
23. 23. Provides in-service education to the members of the patients care team and education to the Community.
24. 24. Provides treatment in accordance with established standards of practice and department procedures.
25. 25. Maintains a safe and clean work area and adheres to facility safety standards, as well as complying with all Infection Control, Universal Precautions and OSHA standards for the healthcare professional.
26. 26. Consistently demonstrates sound judgment in the evaluation, planning implementation and follow up of patient therapy programs.
27. 27. Maintains positive relationships and rapport with coworkers, supervisors, patients, family members and facility personnel and administration.
28. 28. Reports to work on time and coordinates schedule for efficient patient treatments.
29. 29. Assists with and participates in Quality Assurance Program as required.
30. 30. Adheres to established confidentiality standards of the patient, as well as the organization.
31. 31. Projects a positive and professional image at all times.
32. 32. May be asked to travel to participate in home assessments within a reasonable distance from the facility.
33. 33. Maintains passwords for the software required for position and keeps all passwords secure.
34. Performs such other duties as assigned and as required to effectively perform the responsibilities of the position and are in the best interest of Northwoods Rehab.
