Job summary

Northwoods Rehab in Minnesota is seeking a Physical Therapist to deliver and oversee comprehensive PT services for patients, following physician orders. You will coordinate with physicians, nurses and therapy staff to plan and implement treatment, and supervise aides and trainees as part of an integrated rehabilitation team.

As a licensed PT, you will assess patients, set goals, document progress, and advise on home programs while maintaining safety, quality and confidentiality within Northwoods

Qualifications

• Bachelor (or higher degree) of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.

Responsibilities

• Provides and directs physical therapy services to patients under physician orders.
• Initiates referrals when appropriate per Northwoods Rehab policies.
• Conducts regular screenings to determine need for intervention.
• Evaluates patients to obtain data for treatment planning.
• Communicates evaluation findings to patients and families.
• Establishes patient schedules with team members.
• Develops treatment plans with short and long-term goals.
• Coordinates treatment plans with patient, family and team.
• Implements treatment as outlined and monitors response.
• Adapts treatment to meet patient needs and goals.
• Develops home or community programs to maintain progress.
• Documents assessment, treatment, follow-up and termination.
• Ensures patient care aligns with standards and protocols.
• Provides education to patients, families and staff.
• Participates in discharge planning.
• Supervises PT aides, licensed assistants and trainees.
• Reviews quality and effectiveness of PT services.
• Travels for home assessments within reasonable distance.
• Maintains passwords and security for required software.
• Performs other duties to support rehabilitation services.

JOB RELATIONSHIPS

Responsible to and reports to the Director of Therapy/Rehab.

JOB RESPONSIBILITIES

1. 1. Provides and directs physical therapy services to patients, including the assessment, treatment, program planning and implementation, related documentation and communication. Functions under physician’s orders.
2. 2. Provides physical therapy services to patients and initiates referrals when appropriate and in accordance with Northwoods Rehab Policies and Procedures.
3. 3. Conducts screenings of patients at regular intervals to determine need for intervention and /or Treatment.

6. 6. Establishes individual patient schedules in conjunction with other team members.
7. 7. Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
8. 8. Coordinates treatment plan with patient, family and other team members.
9. 9. Implements and conducts treatment as outlined in treatment plan.
10. 10. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
11. 11. Develops appropriate home or community programming to maintain and enhance the performance of the patient in their own environment.
12. 12. Documents results of patient’s assessment, treatment, follow-up and termination of services.
13. 13. Adapts treatment to meet the individual needs of the patient.
14. 14. Provides, recommends, fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of such equipment.
15. 15. Participates in discharge planning.
16. 16. Supervises and directs Licensed Physical Therapy Assistant in the implementation of the treatment Plan.
17. 17. Supervises Physical Therapy Aides, Rehabilitation Techs/Aides, students and volunteers.
18. 18. Reviews the quality and appropriateness of the total services delivered and of individual physical therapy programs for effectiveness and efficiency, using predetermined criteria.
19. 19. Participates in patient, family and staff education.
20. 20. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient.
21. 21. Adheres to Northwoods Rehab procedures regarding documentation of physical therapy services.
22. 22. Attends in-service meetings and facility meetings as required.
23. 23. Provides in-service education to the members of the patients care team and education to the Community.
24. 24. Provides treatment in accordance with established standards of practice and department procedures.
25. 25. Maintains a safe and clean work area and adheres to facility safety standards, as well as complying with all Infection Control, Universal Precautions and OSHA standards for the healthcare professional.
26. 26. Consistently demonstrates sound judgment in the evaluation, planning implementation and follow up of patient therapy programs.
27. 27. Maintains positive relationships and rapport with coworkers, supervisors, patients, family members and facility personnel and administration.
28. 28. Reports to work on time and coordinates schedule for efficient patient treatments.
29. 29. Assists with and participates in Quality Assurance Program as required.
30. 30. Adheres to established confidentiality standards of the patient, as well as the organization.
31. 31. Projects a positive and professional image at all times.
32. 32. May be asked to travel to participate in home assessments within a reasonable distance from the facility.
33. 33. Maintains passwords for the software required for position and keeps all passwords secure.
