Job summary

Total Care Therapy in Findlay, OH is seeking a compassionate Physical Therapist to join our growing team. You will evaluate, develop, and implement individualized treatment plans to improve mobility and independence for residents in assisted and independent living settings.

Responsibilities include skilled PT treatments, gait and balance training, progress reassessment, supervising PTAs, and educating residents and staff on home exercise programs and fall prevention.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics or related settings preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise and gait training.
• Reassess patient progress and adjust plans as needed.
• Supervise Physical Therapist Assistants (PTAs) in line with state rules.
• Document promptly in the EMR.
• Educate residents, caregivers, and staff on home programs and fall prevention.
• Collaborate with interdisciplinary team to deliver coordinated care.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
