Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Findlay, OH. This part-time role involves evaluating residents, developing individualized treatment plans, and delivering skilled PT services.

You will supervise PTAs as needed, document via EMR, and collaborate with an interdisciplinary team to improve mobility and independence for older adults. Requirements include a PT license, CPR/BLS, and a valid driver's license.

Qualifications

• Current State Physical Therapist (PT) License.
• CPR/BLS Certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

About the Role
