Falls City Nursing & Rehabilitation Center in Falls City, NE is seeking a PRN Physical Therapist to join our in-house rehab program. The focused role involves screening, evaluating, and delivering physical therapy services to geriatric patients in post-acute settings.

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans complying with regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan: manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide consultation to patients, families, caregivers and other providers regarding physical disorders.
• Generate discharge summaries of services provided in compliance with regulatory and clinical requirements.

Falls City Nursing & Rehabilitation Center

Falls City Nursing & Rehabilitation Center in Falls City, NE is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.


• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.


• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.


• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.


• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
