Qualifications

• Current Physical Therapist license in the State of Texas.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification or ability to obtain as required.
• Strong clinical, communication, organizational, and interpersonal skills.
• Ability to work effectively with older adults and individuals with mobility needs.
• Previous experience in skilled nursing/rehabilitation is preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions based on residents’ needs and goals.
• Perform therapeutic exercises, gait, balance, mobility interventions.
• Monitor resident progress and modify plans as appropriate.
• Educate residents, families, and caregivers regarding therapy programs.
• Collaborate with nursing, physicians, occupational, and speech therapists and other care team members.
• Complete accurate and timely clinical documentation per facility and regulatory requirements.
• Participate in care planning and communicate changes in resident condition.
• Promote resident safety and independence throughout rehabilitation.
• Maintain professional standards and comply with applicable regulations.

Job Summary

Avir at Del Sol is seeking a compassionate and skilled Physical Therapist to join our team. The Physical Therapist will provide high-quality rehabilitation services to residents, with an emphasis on improving mobility, strength, balance, independence, and overall quality of life.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.

• Provide skilled physical therapy interventions based on each resident’s needs and goals.

• Perform therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.

• Monitor resident progress and modify treatment plans as appropriate.

• Educate residents, families, and caregivers regarding therapy programs and safe mobility.

• Collaborate with nursing staff, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary care team.

• Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements.

• Participate in care planning and communicate changes in resident condition or functional status.

• Promote resident safety and independence throughout the rehabilitation process.

• Maintain professional standards and comply with applicable federal, state, and facility requirements.

Qualifications

• Current Physical Therapist license in the State of Texas.

• Graduate of an accredited Physical Therapy program.

• Current CPR/BLS certification or ability to obtain as required.

• Strong clinical, communication, organizational, and interpersonal skills.

• Ability to work effectively with older adults and individuals with varying levels of mobility and medical needs.

• Previous experience in skilled nursing, rehabilitation, long-term care, or a related setting is preferred.
