Sapphire-Health- is seeking a dedicated Physical Therapist to assess, plan, and implement personalized physiotherapy interventions across inpatient wards, outpatient clinics, and home health settings.

You will help patients improve mobility, reduce pain, and regain independence, while documenting progress in EMR and collaborating with multidisciplinary teams to ensure seamless transitions from hospital to home care.

Qualifications

• Valid licensed Physical Therapist in Pennsylvania.
• Experience in acute care, inpatient hospital settings, outpatient clinics, or home health.
• Strong knowledge of physiology, anatomy, medical terminology, and rehabilitation techniques such as trigger point therapy.
• Excellent communication across multidisciplinary teams.
• Knowledge of discharge planning processes and home & community care services is desirable.

• Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional limitations.
• Develop and implement tailored care plans that incorporate evidence‑based physiotherapy techniques.
• Provide hands‑on therapy sessions focusing on pain relief, injury rehabilitation, and functional improvement.
• Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital to home or community care settings.
• Educate patients and their families on exercises, injury prevention strategies, and home care routines to support ongoing recovery.
• Maintain accurate documentation within electronic medical record (EMR) systems, including progress notes and treatment plans.
• Stay current with advancements in physiotherapy practices by engaging in continuous professional development.
