Job summary

Deercreekseniorliving is seeking a licensed Physical Therapist to provide comprehensive therapy services. The role involves developing personalized care plans and directing treatment while collaborating with patients and their families.

Successful candidates will have at least two years of experience, preferably in home health, and demonstrate excellent communication and organizational skills. Full COVID-19 vaccination or an approved exemption is required.

Qualifications

Job description

DUTIES & RESPONSIBILITIES
  • Assume responsibility for ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provide patient, caregiver, and family counseling and education.
  • Prepare clinical notes and progress summaries based on attainment of goals.
  • Communicate with the physician responsible for the home health plan of care and other healthcare practitioners as appropriate.
  • Provide physical therapy services according to the written physician's plan of care.
  • Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
  • Evaluate the home environment.
  • Assess muscle strength, mobility, gait, ROM, and potential for rehabilitation.
  • Direct physical therapy treatment.
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report to physician patient's reaction to treatment or changes in condition.
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of therapy they may work with a patient.
  • May train patient in the use of prosthetic device.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Participate in discharge planning for the patient and provide in‑service education programs for nursing organization personnel as needed.
  • Participate in peer consultation process.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
  • Supervise Physical Therapy Assistants according to organization policy and state regulations.
  • Participate in the home health agency's quality assessment and performance improvement program and home health‑agency sponsored in‑service training.
JOB REQUIREMENTS
  • Licensed to practice as a physical therapist within the State, if applicable.
  • Passed an examination for physical therapists approved by the state.
  • Possesses a degree from a baccalaureate or master’s program in physical therapy approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • Two years of appropriate experience as a physical therapist; community/home health experience is preferred.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Fully vaccinated against COVID‑19, or have an approved exemption (must be in place by the first day of work).
