Job summary

Victory Home Health – Durant is seeking a Physical Therapist to provide patient-centered care in the home setting, following a defined plan of care with limited supervision.

You will design and adjust treatment plans, educate patients and families, coordinate with the care team, and help patients regain function while maintaining safety and dignity.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Two (2) years of experience as a physical therapist in the last five (5) years.
  • Appropriate competency assessments completed.
  • Current licensure; CPR certification; current driver’s license and proof of auto liability insurance

Responsibilities

  • Treats patients per Plan of Care with ongoing assessment.
  • Designs and implements a Plan of Care for the patient based on assessment.
  • Provides direct patient care and reassesses functional needs to ensure outcomes.
  • Instructs patients and caregivers in home exercise programs and advises on assistive equipment.
  • Maintains patient rights and dignity and completes documentation timely.

Job Summary:

Physical therapists provide treatment for physical impairments, functional limitations or disabilities. The treatment is in accordance with their plan of care. Limited supervision is required.

Essential Functions:
  • Conducts therapy practice within defined standards of care according to APTA guidelines and practice act.
  • Designs and implements a Plan of Care for the patient and significant others based upon a thorough assessment.
  • Provides direct patient care, evaluates/reassesses patient’s functional needs to ensure optimal outcomes, consults with other specialists as needed.
  • Assures continuation of therapeutic plan by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and providing assistive equipment, as needed.
  • Evaluate outcomes of care with appropriate discharge planning.
  • Provides direction and leadership to PTA, aides, and students.
  • Makes visits as scheduled and participates in on‑call rotation as required.
  • Develops coordination and cooperation among other health care disciplines to provide quality patient care.
  • Maintains patient rights and dignity.
  • Completes and submits documentation timely and accurately according to policy.
  • Communicates effectively with patients, families, and the healthcare team.
  • Enhances professional development through participation in educational programs, current literature, in‑service meetings and workshops.
Additional Responsibilities:
  • Performs other duties as assigned or requested.
  • Conforms to all applicable Agency policies and procedures.
  • Participates actively in continuing education and in‑services.
  • Maintains confidentiality of patient information and business trade practices
  • Assumes accountability for reporting incidents and complaints according to Agency policy.
Knowledge / Skills / Abilities:
  • Ability to supervise in accordance with Agency’s policies and applicable laws.
  • Ability to respond to common inquiries or complaints, regulatory agencies, or members of the business community
  • Organizational and leadership abilities
  • Cooperative attitude
  • Advanced written and verbal interpersonal communication
  • Advanced math skills related to patient care.
Information Management:
  • Treats all information and data within the scope of the position with appropriate confidentiality and security
  • Cooperates fully in all risk management activities and investigations.
  • Keeps abreast of changes in health care law.
  • Maintains Agency/program compliance with local, state and federal laws as well as state accreditation standards.
Minimum Position Qualifications:

Environmental Conditions:

Works in patient's home in various conditions, possible exposure to blood and body fluids and infectious diseases. Ability to work flexible schedule, ability to travel locally, some exposure to unpleasant weather. Moderate noise level; tasks may involve moderate stress and emotional demands.

Physical Requirements:

Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and resource acquisition; meet patient/family psychosocial needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.
