Job summary

BAYADA Home Health Care is seeking a Physical Therapist (PT) for full-time or part-time home health visits across Kent County, DE, serving adult and geriatric clients in Dover, Magnolia, and Camden.

New graduates are encouraged to apply. The PT will participate in multidisciplinary care, develop individualized plans, and document care using EMR on a tablet; strong communication and organizational skills are required.

Qualifications

• A current DE Physical Therapist - PT license is required.
• Graduation from an APTA-approved program indicated by transcripts or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Apply protective interventions to prevent wounds and promote healing.
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Review and revise care plans with nursing in the multidisciplinary team.
• Educate clients and family members on rehabilitative care for health and independence.
• Document observations and interventions using a touchpad EMR tablet.

BAYADA by Bayhealth Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full time or part time opportunity performing home health visits to our adult and geriatric patients throughout Kent county, DE. This office services adult and geriatric clients on a per visit basis in territories throughout Dover, Magnolia, and Camden, DE.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualificationsfor a PT - Physical Therapist include:

• A current DEPhysical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independent and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$72.00 - $76.00 per point ($112k - $118k annually)
