Position Overview

We are seeking a Part-Time Physical Therapist who is passionate about helping clients age in place through movement, mobility, and functional wellness. This role is perfect for a clinician who wants to focus on quality of care, personalized outcomes, and practical solutions rather than productivity quotas.

You’ll provide in-home visits across the Des Moines metro area and occasionally participate in community education events. Whether helping someone prevent falls, regain strength after hospitalization, or optimize their mobility in daily life, you will play a key role in helping clients thrive at home.

Key Responsibilities

• Conduct in-home evaluations and provide evidence-based interventions focused on mobility, strength, balance, and fall prevention

• Design person-centered treatment plans that reflect clients’ goals and support their ability to remain at home

• Educate clients and caregivers on movement strategies, adaptive techniques, and home exercise programs

• Collaborate with occupational and speech therapists as well as caregivers, case managers, and community partners

• Support wellness initiatives such as caregiver training, proactive mobility programs, and aging-in-place planning

• Document care accurately and efficiently using HIPAA-compliant systems

• Occasionally participate in community events, workshops, or educational sessions to support healthy aging across the lifespan

Required Qualifications

• Licensed Physical Therapist in the state of Iowa

• Clinical experience with aging adults or interest in working with older populations in a home setting

• Strong interpersonal and clinical reasoning skills

• Comfortable working independently and creatively in home environments

• Valid driver’s license and reliable transportation within the Des Moines metro area

• Availability for weekday home visits (flexible hours)

Preferred Experience

• Knowledge of home safety principles, fall prevention strategies, and caregiver education

• Familiarity with Medicare documentation

• Interest in community-based wellness or nontraditional therapy models

• Proficiency in digital tools such as Google Workspace or mobile EMR platforms
