H2 Health is seeking a licensed Physical Therapist for 1:1 patient care in a senior living community in Lowry. This part-time or PRN role offers a flexible schedule and the chance to thrive within a clinician-led, patient-focused environment.

You’ll evaluate, design personalized treatment plans, document progress in EMR, and collaborate with a multidisciplinary team to improve function and quality of life for residents. New grads welcome; outpatient experience is a plus.

Qualificações

• Must have a degree from an accredited Physical Therapy program.
• Active or eligible state PT license to practice.
• New graduates are welcome; outpatient experience is a plus.

As a Physical Therapist, you will work collaboratively in a supportive environment, using your expertise to create personalized treatment plans and provide compassionate care.

• Education:Must have a degree from an accredited Physical Therapy program.
• License:An active or eligible state license to practice as a Physical Therapist (PT)
• Experience:New graduates are welcome! Outpatient experience is a plus, but we value passion and a commitment to patient care.

Compensation & Benefits:

Pay Range: $48.00 - 58.00 per hour, depending on experience and credentials.
