Job Title: Physical Therapist (PT) – Home Health

Location: Colorado Springs, CO | Full-time or Part-time | CenterCare Home Health

Responsibilities

Your Role

• Evaluate and treat patients in the home setting
• Assist in improving mobility, strength, and safety
• Document patient progress using EMR (Kantime experience is a plus)

Qualifications

• Active Colorado PT license
• At least 1 year of experience in acute care or home health (required)
• Medicare home health experience is highly preferred
• Passion for patient-centered care
• Experience with EMR systems
