Legacy Healthcare Services seeks a Physical Therapist to join our Colorado team, delivering compassionate rehabilitative care to seniors. You will design individualized treatment plans, work within a coordinated clinical team, and help residents achieve their highest quality of life.

This role emphasizes one-on-one sessions, ongoing professional growth, and a Monday–Friday schedule with no weekend or holiday requirements. We support licensure reimbursement and comprehensive benefits.

Qualifications

• Degree in physical therapy or related field.
• Current Physical Therapy license in state of practice, or ability to obtain before starting.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency for documentation and scheduling.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the care plan.
• Treat residents one-on-one to provide individualized attention with a functional-based approach.
• Individualize treatment sessions to meet each patient’s needs.

Pay Range

USD $42.00 - USD $50.00 /Hr.
