Job summary

Legacy Healthcare Services seeks a Physical Therapist to join our team and deliver rehabilitative care to senior residents in our community settings. You will assess patients, plan treatment programs, and provide individualized sessions as part of a coordinated care team.

This full-time role offers a Monday–Friday schedule with no weekends or holidays, plus benefits, licensure reimbursement, CEUs, and opportunities for advancement within a not-for-profit organization serving nearly 500 senior

Qualifications

• Must hold a Physical Therapy license in the state of practice or be able to obtain prior to starting.
• Degree in Physical Therapy or related discipline.
• Excellent interpersonal, fiscal, and clinical skills.

Responsibilities

• Provide rehabilitative services to residents per medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one-on-one with individualized, functional-based treatment sessions.

Position requirements:

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency

Pay Range

USD $44.00 - USD $50.00 /Hr.
