Job summary

Stern at Home Therapy is seeking a PRN Physical Therapist to visit patients in Olean, NY, and surrounding communities. You will provide one-on-one home health PT, maintain clinical autonomy, and coordinate with referrals and scheduling teams.

The role supports a flexible schedule with a minimum commitment of 5 patients and independent contractor status. Compensation includes per-visit and per-evaluation rates, with strong autonomy and administrative support from the team.

Qualifications

• Must hold an active Physical Therapist license in New York.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation and valid license required.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently in a home-based environment.

Responsibilities

• Perform PT evaluations and develop individualized plans of care.
• Provide one-on-one PT treatments in patients' homes.
• Help patients improve strength, balance, mobility, and independence.
• Provide gait, transfer training, and fall-prevention strategies.
• Recommend DME and home safety modifications when appropriate.
• Educate patients and caregivers on exercises and safety.
• Monitor progress and adjust treatment plans as needed.
• Complete accurate clinical documentation and coordinate with care team.

Physical Therapist (PT) - Home Health | Olean, NY | PRN

Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Olean, NY, and surrounding communities.

This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible home health work while maintaining control over when and where they see patients.

With a minimum commitment of just 5 patients, our PRN option provides flexibility while allowing therapists to build a geographically concentrated caseload within the Olean area.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.

You'll maintain clinical autonomy over evaluations and treatment plans while our administrative team assists with referrals, scheduling, authorizations, and patient coordination.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients' homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, therapeutic exercise, and fall-prevention strategies.
• Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.

Compensation:

• PRN 1099 - Minimum 5 Patients.
• $62 per follow-up visit.
• $75 per evaluation.
• Fixed per-visit compensation with no tiered volume model.
• PRN positions are 1099 independent contractor roles only.
