Job summary

Health Advocates Network, Inc. in Chowchilla, CA is seeking a dedicated Physical Therapist (PT) to join our correctional healthcare team.

The role offers a pay rate of $65.70 per hour (W2) with Monday-Thursday shifts 7:00 AM - 3:30 PM, providing high-quality patient care in a safe, professional setting. The ideal candidate will hold a California license in good standing, have at least one year of PT experience, and be prepared to meet ongoing licensing and compliance requirements.

Qualifications

• Current, valid, and unrestricted CA license/certification in good standing.
• Minimum of 1 year of recent experience as a Licensed Physical Therapist.
• Copy of Primary Source Verification (PSV) from the licensing board or verification agency, if required.
• Copy of current required certifications and compliance documents (e.g., BLS, N95 fit test, immunization records, TB test).

Responsibilities

• Evaluate patients to assess mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans to improve mobility and reduce pain.
• Administer therapeutic exercises, manual therapy, and rehabilitation techniques.
• Monitor progress and adjust plans based on outcomes.
• Educate patients and caregivers on exercises and home programs.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain documentation of evaluations and progress.
• Ensure compliance with facility policies and safety standards.

Physical Therapist (PT) Duties & Responsibilities:

• Evaluate patients to assess mobility, strength, balance, and functional limitations.

• Develop and implement individualized treatment plans to improve mobility and reduce pain.

• Administer therapeutic exercises, manual therapy, and other rehabilitation techniques.

• Monitor patient progress and modify treatment plans based on clinical outcomes.

• Educate patients and caregivers on exercises, injury prevention, and home exercise programs.

• Assist patients in restoring function following injury, surgery, or illness.

• Maintain accurate and timely documentation of evaluations, treatments, and patient progress.

• Collaborate with physicians and interdisciplinary healthcare teams to coordinate care.

• Ensure compliance with facility policies, safety standards, and regulatory requirements.

• Promote patient independence and improve overall quality of life through rehabilitation services.

Minimum Qualifications

• Current, valid, and unrestricted CA license/certification in good standing, as required for the position.

• Minimum of 1 year of recent experience as a Licensed Physical Therapist.

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
