At National Church Residences, Home Health is where our mission meets people in the place they most want to be, their home.

Our Home Health team brings skilled nursing, therapy, education, and support directly to patients, helping them recover from an illness, injury, surgery, or hospitalization while remaining safe and independent. Because we care for patients in their own environment, our clinicians can see the whole picture, including daily routines, caregiver support, safety concerns, and barriers that may not be visible in a traditional healthcare setting. What's really cool is that this is one of the only healthcare settings where you can be one-on-one with the patient, whereas in the hospital, SNF, or IRF, you'll be caring for an entire hallway or group of residents/patients.

What makes our team special is the way we work together. Nurses, physical therapists, occupational therapists, speech-language pathologists, social workers, home health aides, clinical leaders, schedulers, intake specialists, and quality professionals each play an important role in the patient's success. Although our clinicians work independently in the field, they are supported by an experienced and collaborative interdisciplinary team.

Our Home Health is located in both Central and Southern Ohio, with a team of health care professionals that is unmatched in the area! We are the nation's largest provider of affordable senior housing and health care services, and we are seeking a Physical Therapist who will share in our vision to advance better living and care for seniors!

Qualified candidates for this position offer:
  • Education: Doctorate, Master or Bachelor of Science Degree in Physical Therapy.
