We are hiring for a Full‑Time Home Health Physical Therapist / PT to join our passionate team in Mansfield, CT.

Responsibilities

• Assess and evaluate patient care needs related to functional status, mobility, endurance, pain, and other medical conditions.
• Determine a treatment plan, perform interventions to improve and enhance patient well‑being, and evaluate patient progress.
• Evaluate the patient's functional status and physical therapy needs; assist the physician in developing the therapy plan of care.
• Observe and report patient responses to treatment and changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
• Instruct and inform the health care team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Education & Experience

Licensure Requirements

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver's license with vehicle insurance and reliable vehicle access.
