Job summary

CareCore at Willowood in Brunswick, OH is seeking a PRN Physical Therapist to join our in-house rehabilitation team serving residents in a skilled nursing and long-term care setting.

You will perform comprehensive evaluations, develop individualized plans, and deliver evidence-based therapy to improve mobility and independence while collaborating with physicians, nursing staff, residents, and families. Net Health and PCC are used for documentation.

Qualifications

• Current Ohio PT license in good standing.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing or long-term care preferred.
• Strong communication, documentation, and clinical decision-making skills.
• Experience with Net Health and PCC is preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based therapy to help residents improve mobility, strength, balance, and functional independence.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families.
• Complete accurate and timely documentation using Net Health.
• Utilize PointClickCare (PCC) as part of the interdisciplinary care process.
• Maintain compliance with all federal, state, and company regulations.

CareCore at Willowood is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our exceptional in-house rehabilitation team. Position Highlights
