Job summary

Jobot is seeking a Physical Therapist for a permanent, on-site position in Boonsboro, MD. The role focuses on evaluating patients, developing individualized treatment plans, and delivering skilled physical therapy within a long-term care setting.

You will collaborate with nursing and medical staff to support positive outcomes, monitor progress, and educate patients and families on exercises and safety.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Maryland Physical Therapist license or eligibility to obtain.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• New graduates may be considered depending on facility needs.
• Strong clinical assessment and treatment-planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Evaluate patients' mobility, strength, balance, and rehabilitation needs.

Job Details

The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide skilled therapy services based on each patient's needs and goals. You will work closely with nursing, physicians, therapy professionals, and other members of the interdisciplinary team to support positive outcomes and successful recovery.

Responsibilities

• Evaluate patients' mobility, strength, balance, functional abilities, and rehabilitation needs
• Develop and implement individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, balance activities, and other appropriate interventions
• Help patients improve strength, range of motion, coordination, endurance, and overall functional independence
• Monitor patient progress and modify treatment plans as clinically appropriate
• Establish measurable goals and provide ongoing assessment of rehabilitation outcomes
• Educate patients, families, and caregivers on exercises, mobility techniques, safety, and continued care
• Recommend and train patients on the appropriate use of assistive and adaptive equipment
• Collaborate with physicians, nursing, occupational therapy, speech therapy, and other members of the care team
• Complete accurate and timely evaluations, progress notes, treatment documentation, and discharge planning
• Participate in interdisciplinary care planning and patient conferences
• Maintain compliance with professional standards, facility policies, and applicable state and federal requirements
• Promote patient safety, independence, and quality of life throughout the rehabilitation process

Qualifications

• Current Physical Therapist license in Maryland or eligibility to obtain
• Previous experience in skilled nursing, rehabilitation, long-term care, or a similar setting preferred
