Job summary

CommuniCare Rehab at Mercer Healthcare Center is seeking a Physical Therapist (PT) for a full-time role within our in-house rehab team. You will evaluate residents, develop treatment plans, and supervise Physical Therapist Assistants to ensure high-quality care.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient/resident charts and develop a treatment plan.
• Implement treatment plan and educate the plan of care to the rehab team and supervise PTAs.
• Attend/Lead interdisciplinary meetings and provide staff/family training when appropriate.
• Opportunity to work at multiple centers to serve a wide variety of patient needs.

Qualifications:

• A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient's/resident's medical charts, evaluations under physician order and develop a treatment plan in accordance with patient's needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
